The following forms may be required for employees at some locations:
 
Employee Performance Appraisals provide for the periodic exchange of information about accomplishments and problems on the job.  On an annual basis, employees must be given formal appraisals and the documentation placed in the employee's offcial personnel file.
There are three appraisal formats - Employee, Mid-Level and Managerial.  The appropriate form should be used, depending on the position.
 
Workers' Compensation insurance is provided for all diocesan employees.  All work-related injuries or illnesses must be reported to the Human Resources Office on the North Carolina Industrial Commission Form 19.
